As a leader in hospital readmission mitigation, we have created our Welcome Home program to help the hospitals in our area achieve their goals of reducing unnecessary readmission but more importantly, helping people stay in the comforts of their own homes.

Welcome Home helps individuals transition back home after an outpatient surgery or stay at the hospital or rehab center. These services are also available for those who simply need a little extra help getting home and settled.

Take the worry out of the transition with our transitional care specialists, trained caregivers who will transport you home, if needed, and ensure that you are comfortable, safe and well taken care of. We want to provide a sense of confidence as well as reduce your chances of being sent back to the hospital for the same illness or condition.

    4 Hours of Care

    Welcome Home is a four-hour minimum service. Adults living alone or who require help as they recover from medical procedures or long term hospital stays can get assistance transitioning comfortably to life at home. Caregivers are dedicated to reducing readmission risk by ensuring you have convenient access to the things you need to heal.

    Grocery Shopping

    Let our caregivers handle the groceries while you recuperate. Avoid doing too much too soon as we stock the pantry and the fridge with healthy and delicious snacks and ingredients.

    Transportation

    While your ability to get out and about is limited, Georgetown Home Care provides assistance with transportation through a partnership with Uber Health that allows caregivers to accompany you as you run errands or attend appointments or events.

    As an additional affordable option, certain caregivers are available to provide transportation using their own vehicles.

    Meal Preparation

    We offer meal preparation as part of the range of services our highly-qualified and trained caregivers provide. Get assistance so that you can enjoy nutritious, home-cooked meals as you recover from your hospital stay.

    Bathing, Grooming, and Toileting

    Avoid common senior injuries while entering and exiting the bath and shower. Caregivers can also perform bathing rituals as necessary, as well as help with hair and oral care, application of cosmetics and other toiletries, trimming of nails and stray hairs, and assist with toileting and adult diaper changes.

    Medication Reminders

    Our caregivers can provide medication reminders upon request, which may also include assistance with placing and picking up prescriptions, as well as scheduling appointments to renew, refill, or review medications.

Hospital readmission is a serious problem in the United States, and it is by no means a new problem. There are many facts and theories about why hospital readmission has become such a major issue, including a lack of literacy with regard to different health plans, such as those offered by the Affordable Care Act or Medicare/Medicaid services.
