Our experienced caregivers offer a range of services that include:
Welcome Home Program (Transition From Hospital Stay)
Getting home from the hospital can be a little stressful for everyone involved. Our caregivers can provide three different methods of transporting your senior loved one back to their home: accompany them in an Uber Health services vehicle, drive them in the caregiver’s vehicle (the client will be charged for mileage), or the caregiver can drive the client’s own vehicle.
As a part of our Welcome Home Program, the caregiver will also provide or facilitate:
- Helping the client to get changed, comfortable, and situated
- Grocery pickup to fill the fridge and pantry
- Prescription orders/pickup/refills
- Medication reminders
- Laundry and linen changes
- Light housekeeping for appearance, health, and safety (see more below)
- Assistance with bathing and toileting
- Companionship
When combined with longer-term home care, as needed, these services significantly reduce a patient’s risk of hospital readmission.

★★★★★
“We started with the minimum hours per week, and I saw how my sister thrived with this stellar home care and companionship. When her condition began to deteriorate, we increased care hours to five days a week, and her caregiver was responsive and essential not only to my sister’s increasing needs, but to my own ease in knowing she was well taken care of.”
